Surgery Reservation Policy
1. A $600 deposit is required to secure the time allotted for your procedure. We cannot reserve a surgery appointment for you unless the booking fee is received in Zelle, cash, cashier’s check, money order, or an authorized credit card number.

2. Cancellation Policy
You need to give us fourteen (14) days in advance notice to cancel or reschedule your surgery appointment. If the notice is given in less than 14 days, $500 will be charged to your account and is not refundable. If we do not receive any notice from you, $600 will be charged and is non-refundable.
The deposit will be credited to your account and the remaining balance of the surgery fee is payable by cash, cashier’s check, or credit card on the day of the procedure.
If we cannot proceed with the surgery due to your medical condition on surgery day, we will give you a 100% refund on the deposit.
